WAIVER AND RELEASE OF ALL CLAIMS

Be aware that in signing up and participating in goat yoga at the HAWS Schallock Center for Animals, you will be expressly assuming the risk and legal liability and waiving and releasing damages or loss which you, your minor child/ward might sustain as a result of such attendance, participation, rental or use of the facility.

HAWS, the HAWS Schallock Center for Animals, its members, officers, directors, employees, agents, volunteers, and contractors (collectively the HAWS Schallock Center for Animals) are committed to conducting its events, activities, and renting its facility, in a safe manner. The HAWS Schallock Center for Animals strives to reduce risks and insists that all participants follow safety rules and instructions designed to promote safety. However, participants and parents/guardians of minors registering for goat yoga must recognize that there is an inherent risk of injury or death for participants, when participating in goat yoga.

You are solely responsible for determining if you, your minor child/ward is physically fit and/or adequately skilled for goat yoga. It is always advisable, especially if the participant, child, ward suffers from any underlying medical condition, is pregnant, or has recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

In consideration for my participation in the Activities:

I have chosen to participate in goat yoga provided by HAWS at the Schallock Center for Animals. I acknowledge that I understand the nature of the activities that I will be participating in and the possibility that despite precautions, accidents, illness, or physical injury may occur.

In consideration for my participation in goat yoga: I recognize and acknowledge that there are certain risks of physical injury or death to participants, and I agree to assume the full risk of any and all injuries, death, damages or losses, regardless of severity, that my minor child/ward, or I sustain as a result of goat yoga. I further agree to waive all claims I, or my minor child/ward, may have or which may accrue to me or to my minor child/ward as a result of participating in the goat yoga against the HAWS Schallock Center for Animals.

I do hereby fully release and forever discharge the HAWS Schallock Center for Animals from any and all claims for injuries, death, damages or loss that my minor child/ward, or I may have or which may accrue to me or to my minor child/ward or to my family, my estate, my heirs and/or assigns, arising out my/our participation in goat yoga;

I have read and understand the above information, warning of risk, assumption of risk, and waiver and release of all claims, and have signed this Waiver and Release freely and knowingly. By registering on-line my payment submission shall have the same legal effect as my original handwritten signature on this form; and

I acknowledge that the activities and/or the HAWS Schallock Center for Animals may be photographed/videotaped by the HAWS Schallock Center for Animals for promotional purposes. By participating in the Activities, I grant permission for my/our image(s) to be used for such purposes.

This WAIVER AND RELEASE OF ALL CLAIMS is acknowledged by checking “I have read and understand the rules, terms, and conditions” box on the registration page. By submitting payment, I consent to the release authorization.
