HAWS Parent Agreement Document / Kindness Pledge

 

IN PERSON PROGRAM TERMS:

Payment and Registration

There will be a 30% administrative fee for all cancellations. There will be no refunds or transfers given within two weeks of the start of the program session.

Emergency Contact Information/ Pick-up Permission

It is very important to HAWS that we protect the safety of all campers. For this reason, only adults over the age of 18 who are on your camper’s "pick-up list" will be allowed to pick up your camper. We ask that when you register your child, you list on the form any adults who will be picking up your child. If during the week you need to add someone to that list, you may do so. We reserve the right to check ID of anyone picking up a camper to make sure that his or her name is on the pickup list.

Children Information

It is important to inform HAWS of any special needs; physical, medical, mental or emotional, that your child may have so that we can provide a successful HAWS experience. Failure to inform HAWS of any of these needs that are necessary to know when instructing your child may result in your child being unable to participate and a loss of program fees. 

If any of this information changes, I will inform HAWS staff of these changes.

Cameras/Phones/Electronic Devices 
 
Phones and handheld electronic devices are permitted at guardian's discretion, but all devices (including phones) must remain silent and put away during camp lessons and activities. Campers may be permitted to use their phones during lunch, snack, and extended care hours only. Cameras (or phones that may be used as cameras) may be carried, but can only be used when instructed by a staff member. Photos taken behind the scenes may not be posted on social media, websites, blogs, etc. If at any time the camera, phone, or handheld electronic device becomes a distraction or is not being used appropriately, campers will be asked to put it away and not use it for the remainder of the day. If the item continues to cause a problem, HAWS camp staff reserve the right to collect the item and return it at the end of the day. We ask that larger electronic devices – such as laptops and iPads – stay at home. 
 

I understand and agree to the following

- In the event of an emergency, I give permission to HAWS to seek emergency care by the medical facility selected by HAWS to hospitalize and secure proper treatment for my child(ren). 

- I consent to my child’s participation in all activities and trips, which are part of the program and under the direction of HAWS staff.

- I give permission to HAWS to use photographs and video of my child in publicizing and promoting HAWS’ work.

- I consent to the minor participating in programs at the Humane Animal Welfare Society of Waukesha County, Inc., "HAWS," and all of its activities and programs. The undersigned, for herself or himself and on behalf of said Minor(s), does hereby absolutely and unconditionally release, indemnify, hold harmless and forever discharge HAWS, its employees, successors, assigns, and agents and each of them, from and against any and all claims, demands, obligations, and liabilities of every nature and kind whatsoever including, without limitation, negligence, occurring during, directly or indirectly resulting from or arising out of the minor’s participation in such programs. As to matters covered hereby, the CONSENT AND RELEASE shall extinguish all claims, demands, and rights which the undersigned or the minor (and/ or each of their heirs, successors, and assigns) has or may ever have against the parties released hereby, or any of them, for any injuries, costs or damages to the Minor occurring during, directly or indirectly resulting from or arising out of the Minor’s participation in such programs whether such injuries, costs or damages are known or unknown, foreseen or unforeseen, ascertainable or unascertainable.

- Notice: A person who is engaged for compensation in the rental of equines or equine equipment or tack or in the instruction of a person in the riding or driving of an equine or in being a passenger upon an equine is not liable for the injury or death of a person involved in equine activities resulting from the inherent risks of equine activities, as defined in section 895.481(1)(e) of the Wisconsin Statutes.

By registering your child and submitting payment to HAWS, you agree to the terms as they are listed. 

 

KINDNESS PLEDGE: 

- While attending my program at HAWS, I pledge kindness, empathy and compassion.  

I pledge to…  

  • Listen to and respect my educators 
  • Always include others 
  • Use only kind words towards peers and adults 
  • Help others when they need help
  • Be kind towards other staff and volunteers at HAWS
  • Respect the building and animals' cages
  • Follow instructions given by educators to stay safe, especially around animals
  • Most importantly, always be kind and loving towards all animals at HAWS 

I understand and acknowledge by signing this pledge, I am responsible for my actions.  If at any time a HAWS instructor deems my behavior is not reflecting this pledge I made, they will inform me and my parent or guardian and can decide whether or not I will be allowed to continue to participate. VIRTUAL PROGRAM TERMS (school year only):

As the parent/legal guardian of the child(ren) named herein, I hereby consent that said child(ren) may participate in the activities including but not limited to activities through online platforms and crafts and activities guided by staff from the Humane Animal Welfare Society (“Virtual” programs”), and hereby release the Humane Animal Welfare Society from all liability for any injury or illness that might occur to child(ren) during the course thereof. I understand that Virtual programs cannot substitute for supervision of my child and agree to retain full responsibility for my child during virtual program hours. I understand that the activities included in virtual programs, like any activities of this type, carry the risk of harm and injury. For example, some supplies and equipment for craft activities can cause injury if used improperly. I understand that there are risks connected with the type of on-line learning platforms used for Virtual programs, including but not limited to the risk of data mining, phishing, viruses, malware, data breach of online information, and digital footprint. I give my child(ren) permission to sign into the digital platforms used for Virtual programs using the E-mail address I provide. I know of no medical or other condition that would prevent my child(ren) from full participation in activities led by the Humane Animal Welfare Society. I agree to indemnify and hold harmless the Humane Animal Welfare Society for any and all claims and demands.
